For Our Dentist Partners, We may disclose and otherwise use Our Dentist Partners’ names.  We may also collect information from You regarding which of Our Customers were referred to Us by You.  However, We will not sell or otherwise disclose Your customer list to any of Our Dentist Partners.

In order to purchase any products or services on Our Website, You will be required to submit certain contact information, including, but not limited to, Your name, email, phone number, shipping and billing addresses and payment information, to Our third party payment processor.  Payment information may include Your credit card and/or online check information.  The payment processor will collect and store Your payment information and transmit Your other contact information to Us so that We can fulfill Your order.  We will not receive any payment information from the third party payment vendor.  We will store Your contact information and may contact You at any time with future news, advertisements, survey requests and updates, unless You choose to-opt out, as described below.
